Crucial Recruiting is partnering with a pre-seed, post-revenue vertical software startup building the operating platform for elective healthcare practices. The product runs revenue and staffing, converts more of a practice's inbound leads, and replaces a stack of disconnected tools with one system, with AI doing real work inside it rather than sitting on top as a feature. Demand is ahead of the roadmap, customers keep asking for things that do not exist yet, and new accounts are onboarding as fast as the team can take them.
About the Opportunity
This is a founding engineering seat with real equity, and you would own technical direction for entire product initiatives rather than tickets inside someone else's design. The title says full stack and it means it, but backend is where this role lives: architecture, data flows, integrations, infrastructure. You decide where the boundaries go, and you are also the one paged when they break. There is no platform team behind you, and there will not be one for a while. The person who fits is often a tech lead at a larger company who keeps turning the promotion down, because they would rather build the system than sit in meetings about the system.
What You'll Do
Architecture & Systems
- Own technical direction for large product initiatives end to end
- Make the architectural calls on what goes where, and live with them
- Design the data flows and service boundaries as the product surface expands
Build & Integrations
- Ship across both halves of the stack in production: TypeScript/Node on the back, React/Next on the front
- Build and maintain third-party integrations, real-time telephony, and asynchronous pipelines
- Wire AI into the middle of the product, not around the edges of it
Operations
- Run your own deploys, monitoring and incidents. The infrastructure is yours
- Set the reliability bar for a system practices depend on to run their day
AI-Assisted Development
- Direct coding agents well: knowing what to build, when to throw out what the agent produced, and how to run several in parallel without the output degrading
- Ship real LLM work, meaning evals, agents and the fortieth round of prompt iteration, not demos
What We're Looking For
- Typically 6-15+ years of professional software engineering experience
- You have owned architecture on production systems and made the hard calls yourself
- Systems design is where your head goes by default, and backend is where you are happiest
- Genuine production experience in both TypeScript/Node and React/Next, not side projects
- You run your own ops: deploys, monitoring, the occasional late-night incident
- Integrations do not intimidate you: third-party APIs, real-time telephony, async pipelines
- You have shipped LLM features that real users touch
- Strong judgment directing AI coding agents on a team that has gone all-in on agentic development
Technical Environment
- TypeScript/Node monorepo
- Next.js deployed on Vercel
- Backend running on GCP
- Agentic coding tooling used daily across the team
